| 2. Every field must be filled out completely before submitting this application to the Oklahoma Tax Commission. 3. Once you have completed the application form, PRINT a copy and sign it. |
| 4. Make a photocopy of your current Elks membership card. |
| 5. Make your check or money order payable to the Oklahoma Tax Commission. Your Driver's License number must be on it. |
| 6. Send both the completed application and the photocopy of your membership card along with your payment ($7 for new or $6 for renewals) to: |
